Residency recommendation request forms

Do all residency programs require these? For instance, if it is not listed on their website, do you just send a generic ASHP recommendation form for your letter writers? Thanks

E-mail the program and ask - that's what I've done. A few of the programs I'm interested in have their own special forms, and another one doesn't want me to use the ASHP form, but instead wants detailed letters from each reference. At the very least, e-mailing the program will get you on their radar, which is not a bad thing.
