Programs with high surgical volume?

I'm curious if anyone knows which programs have especially high surgical volume, lots of cataracts, etc.

I'd imagine that most are in big cities, but if there are some that are not, please post (I'm not a big city guy).

Some of the programs I know with very high volumes are: Utah, USC, and Southwestern. All are in fairly large cities.

If you're a small town person, then consider Iowa, Madison, and Duke.
