Podiatrist offers "in house physical therapy"

There is an irony here. FiveO's greatest concern is that each specialty should stick to it's own turf, and he is obviously upset that MDs, DPM's, DO's may offer physical therapy services, even when provided by a licensed PT.

Yet this morning I received a newsletter from a local hospital highlighting the PT department. In the article it stated there is no need to go to a specialist for "over-priced" orthoses, since the PT's are now providing custom foot orthoses at their center at a discounted price.

Is there a contradiction here? I didn't know that PT's were experts in lower extremity biomechanics, orthoses, etc., anymore than I'm an expert in PT.

So, you can see that you can't point your finger at the DPM's and blame them for the world's trouble. There's plenty of blame to go around.

Are you feeling ok? PT's have been providing custom foot orthotics for years, that is not exclusively within your scope. Something pretty much everyone doesn't understand (including yourself) is that there is often overlap within professions, but because you do something that is similar to a PT, that does not mean you practice or provide PT, or are even remotely an expert in PT. I wonder how many "disasters" you've caused by recommending to patient's they get your custom orthotics and then they come and see someone like me and tell me how much worse it made them feel. That's ok though.

Did the hospital you're referring to say they provide podiatry on a PT website, or did a PT imply or explicitly state that? I didn't think so. Once again, care to address why you/your colleagues 99% of time do not include their rationale for their "PROM only" etc, genius commentary? How come you think PT's have to call you about their POC to make recommendations about their POC? Sorry, but that doesn't make sense.

PT's are experts in everything. Nobody else knows anything about rehabilitation. Just ask him. He looks down on every profession and even looks down on PTs who aren't doctors...
 
Can I just assume your lack of response to my proposed hypothetical is yes? Because I'd work for a port-o-John if it paid me a million a year...

A million dollars up front out of your pocket with the contract stating that I am in full control of PT (zero exceptions), any episodes that demonstrate otherwise make my year commitment to you voided, and I keep the million dollars. Then I would do it. The problems with POPTS is that physicians refer to "their" PT's more than they should and the PT's are pressured to follow the "orders." If the PT was in full control, it would never be an issue.

I attempted to be diplomatic, and thought that maybe you had come to your senses and would be civil. But AGAIN you make a personal attack on ME and fabricate some scenario about the orthoses (not orthotics) that I've made causing a disaster.

Have I attacked your integrity, skills or credibility? The answer is a strong "no", so I will recommend you refrain from doing this again. At least act like a professional.

You perseverate on the same point and don't seem to pay attention to anything I wrote. I simply stated several times that IF I have a concern and write for a particular caution, I'd like a call from the PT if he/she disagrees. If you have a problem with that, you've simply got a problem and some deep authority issues.

As said several times, no recommendations I make or other docs make are meant with any malicious intentions, so if you take it that way it's really only your perception.

You have attacked me several times, insulted me and have not been diplomatic at all. In reciprocation I will do the same to you. What are you going to about it? And stop trying to "show me the way" to be a professional or whatever it is you're trying to do because I would never ever take your advice considering some of the things you yourself have said. I'll make sure to let my referral sources know about your expert assessments of me.

No no no. You would work FOR a pod. He/she would use you at his/her discretion and tell you what they wanted done. Obviously, if you disagree with the podiatrists plan, you can discuss it with him/her, but ultimately you would be their employee and you could quit if you truly felt you were being harmful to your patients.

It's easy on the internet to say you would reject this scenario, because you think working for a podiatrist is a sign of the pending apocalypse, but IRL I really don't think you would turn this offer down. Why? Because you, not unlike the podiatrist, are offering a service to a patient, and just because you are working FOR someone (as the majority of americans do) does not mean you are being unethical in your treatment.

I agree. But you should stick with realistic scenarios for realistic outcomes or any ethical point driven would have little to no weight. There are enough fallacies in trying to relay a realistic message.

I am a DPT and now a DO. I am defiently going to own a PT clinicn with self referral. Ill just do a quick PT eval and send them back to my PTAs. Quick and easy way to bypass the whining about POPT clinics. It will be a PT owned clinic...
