As an intern, it so often felt like the world was in our shoulders. Not sure if someone's culture has grown out yet? Intern's fault. SCD machine MIA first thing in the morning when everyone has just arrived? Blame the interns. It was also our collective responsibility to do and know everything. I'd love to hear thoughts on general PGY-2 expectations and things to focus your time on, as it seems a bit more nebulous than it did as an intern.

It's going to be highly dependent on how your program is structured. Are you the mid-level on a service with interns? Are you still the most junior resident on a service without interns? Are you an ED consult resident or ICU resident?

Basically, I expect that a 2nd year has moved beyond being a reporter of information. When you talk to me about an issue, I expect that you have a plan for the next step. Or when you present a consult, I expect you have a differential and a plan for working it up. In appropriate circumstances, I may expect that you've independently taken such "next steps". On a service with interns, I also expect that you've started to assume some supervisory responsibility. For all those things that you mentioned, the interns should have them done...but the 2nd year should make sure the interns understand those are their responsibilities. But what I also look for is that you still know when to ask for help. Second year is perhaps the most dangerous year since many have fallen victim to the idea that they know more than they actually do.

Add all this on top of being more engaged in the operating room...

As an intern, when you know things, or do things correctly, people are occasionally impressed.

As a PGY2 they arent, ever.

Good luck!
