You might also try finding the 3-volume Fonseca Oral & Maxillofacial Surgery Text. Turvey is a main co-author and well-respected authority on orthognathics. The book set should also cover some of surgical treatment of other deformities as well as surgical approach. If you want to be well-versed in surgical approach, however, you prob want to buy Ellis/Zide's book: Surgical Approaches to the Facial Skeleton.

Posnick will be coming out with a comprehensive orthognathic text possibly as soon as next year. I suspect it will become the bible.
