I saw that someone posted about IVs from Tufts but I also emailed the PD as I am waiting to hear and he said they are still reviewing and plan to extend IVs till Oct so

I am from a community program in IL and my PD sent us this email. I am on a J1 and tbh I don’t want to take a chance by declining yet. At 16 IVs, applied to 36...hoping for ~20 or so.

Are they talking specifically about ID or fellowship programs in general? I've heard it's true for a lot of fellowship programs this year (see Pulm/CCM as a perfect example). I get your reservations and wish you luck on 16 interviews. I don't know how tired I'll be after 10... I don't think I could imagine doing 20 interviews realistically.

She said it was across the board. Yeah idk 20 might be overkill but idk at least for now they are spaced out and couple are like only 2hrs.
 
Wow sorry to hear that.

More bummed out. Thankfully other interviews went well. But I was excited that since I was going there I'd get a tour/presentation or meet more faculty/whatever.

I start with BIDMC on Tuesday. I feel like a lot of my questions are going to revolve around work-life balance, the clinical experience (good variety, reasonable numbers, diverse patients), research protected time and funding, program priorities, global health opportunities. I interviewed for research pathway programs several years ago and I found that a lot of institutions have these instructor positions to allow time to apply for extramural funding. In that role you are an attending physician but not full faculty and they give a lot of protected research time for a couple of years. So I will ask a bit about those options.

Also, I am Med/Peds and several of my interviews will be for a combined 4-year program with Peds so will ask questions about how they integrate the programs.
Late reply here but wanted to say good luck. I'm combined ID as well and loving the experience (and ignoring the length of the program)!

This is all opinion -
Not really. And the candidate to position is a very poor indicator of chances. Let's remember that every year people apply for ID as backups and not as an intention of doing ID. And that every year there are a significant amount of open spots after the match going on 3-4 years. At this point, I'd speculate that although there may be more candidates, it's only because some people are just applying everywhere/anywhere with no intention of following up/going through.
IE - Candidates applying for Cards or Crit but adding ID as backup because reasons
Or people going into ID to try to backdoor into crit with no intention of ID
Or there is an increasing genuine interest in ID this year.
Who knows
But it doesn't really hurt your chances. You still HAVE to interview with staff and show that you want to do ID. So, if you want to do it, great. If you don't...it'll obviously show. ID is one of the rare specialties where you can't bull**** your way through the door.*


* to clarify - it's really hard to say you want to go into a specialty that pays less and works more than a hospitalist on average without having a genuine interest or passion for the field.

I keep running into same people on my interview trails... I think we all applied to more than what we would have if we had to travel...

I attended 12 interviews, have 3 more to go but of these 12 I attended will not rank 5 for various reasons so I will end up with max 10 to rank which should be OK. Are you sending thank you emails to programs?

Can you clarify? Are you taking Step 3 this year or not? They just need you to pass Step 3. I don't see anywhere that you can't match without step 3.
Otherwise, why would they interview you?
 
A few programs I interviewed at, they were all very good and very much similar (even the city and kind of atmosphere). What things are you guys considering to determine which one is better than the other? :D

I had the same feeling from many programs, but actually, there are many differences if you check the details. For example, I had a preliminary ranking before going to the interviews, but now my top choice is a program that I didn't know almost anything about, but I was fascinated that they offer almost everything I am looking for. I am sure that many of us are doing this, but I am doing an excel file where I write the different aspects of the programs after the interview, including clinical stuff (caps? on-call schedule? EMR? the number of fellows? tracks? good mycology lab? global health? TID?), research (available basic, translational or clinical research? any research training available? potential mentors?), education (academic position in the med school available?), city (good schools? cost of living? friends or family around?) and how I felt before, during and after the interview while interacting in the pre-interview dinner, or with the interviewers. I've done the same. And, honestly, thanks to this zoom environment, I've been pleasantly surprised. Almost all interviews went great and I've had no regrets (for the most part). And, like you said, I've had a couple programs high on my list that I wasn't anticipating.
I'm not into research. I'm into inpatient, acuity, infection control and antibiotic stewardship. And they're really receptive to it. I've actually enjoyed the interviews.
Honestly, I can say I've never had an easier time talking with people about why I want something. This was loads more comfortable than residency.

I myself had a couple of case reports and literature reviews, poster presentations along with 2-3 ongoing clinical projects. I think you have plenty of time to make your profile for competitive ID places. Good luck!

Personally, case reports and research only helps. But what I've found is being involved with Antibiotic Stewardship helps a LOT. Especially with an interest in ID. So, if you can be involved, do it. It speaks a lot about your interests in ID. Even if you don't have a longterm interest. It makes them know you like ID and understand the importance of the field.

But, also, Antibiotic Stewardship is important. So many people forget to stop antibiotics or follow up cultures or consult ID. It's important. I'm biased.
 
Im interested in public health/epidemiology research. Trying to compare Duke vs UNC. Looks like I didn't take as many notes as I would have hoped. Anyone have any thoughts on strengths/weaknesses? Obviously similar quality of life outside of the hospital.

Duke: T32s. 6wk rotations w/ 90-150 consults per rotation. Note cap @ 5. Weekend coverage 1 of 4wks. +global health. +MPH coursework (via UNC)
UNC: T32s. 2wk rotations w/ unknown # consults per rotation. Note cap @ 5. Rotating black/golden weekend. + global health. +MPH.
 
What are you guys’ approach to post IV communications. I am hearing from multiple people that if you’re now in training having your PD directly reach out to your #1 program PD is a long standing tradition. Any thoughts?

First, your program director should've already written you a letter. And spoken about how good you are.
Second, your interview with the should give you an idea about how much you like each other
Third, if you really liked them, then sending an email a couple days later to thank them and say how much you like them would help.

Imo, having a letter from the PD helps enough. You shouldn't really need them to call the other program post interview unless something happened. Either you didn't get to say what you want or bad connections.
But, in the end, it's about selling yourself.

This is my opinion and I might be wrong.

BUT if you had a good interview and enjoyed the talk with staff? Don't ask your PD to email them. Email them yourself. Tell them how much you liked their program and would love to be there.
 
What do you make of post-interview communication from programs stating you will be "ranked highly" or "ranked to match"? Do we take these with a grain of salt? And does the absence of any post-interview communication from programs mean anything? (I know some programs said specifically we would not hear from them after the interview day unless we personally reach out.)
 
I think it depends. I got some messages like those from places where all the communications between the program and me during and after the interview were consistent with it, so I think they are true. On the other hand, I have talked with different ID fellows who told me that they never got those messages from the places they are currently doing their fellowship training, so if the program doesn't send you "the love letter" it doesn't mean they will not rank you, and actually, they may not communicate with candidates due to their institutional policy. Bummed out, match didn't work out this time. Strange... Blame it on my app, interviews? Or Zoom and covid?

I immediately called the program saying how excited I was. Then promptly started looking at places to live lol. Can't wait.

I’m applying to ID next year. I’m an IMG from small community program with a good CV. The problem is the ID service is fairly small and there is only one attending. I’m very involved in the antibiotic stewardship committee (developed some in-house guidelines) and the sepsis committee (developed code SEPSIS). The Abx stewardship committee is led by a PharmD and she is very involved in the sepsis committee too.

So my question is: What do you think about getting a LoR from her (the PharmD)? She could definitely talk about my work in both committees in a positive way but I’m not sure how a PharmD LoR would play out.

I would get the PharmD letter if you think it will be a good letter. As long as they can speak to your desire to do ID fellowship and they have a terminal degree, I think anyone will do. You do need others, obviously, like your PD and maybe a general internist, public health person, critical care or other specialty that can speak to your plans and potential.

For reference, I had 4 letters. 1 from my PD, 1 from my clinic preceptor (general medicine), 1 from a PhD-only, 1 from an ID physician. I interviewed at many of the top programs (Partners, Emory, UWashington, Hopkins, Penn, BIDMC, Vanderbilt, etc.). Having a lot of ID letters is not critical.

Yeah,
Mine were 2 ID, 1 critical care and program director. Keep in mind these were the people i worked closest with during residency. You can partly thank COVID for that.
Having the PharmD would be good if they can speak about you personally. Anyone can write a LOR. But having a good LOR is even better. So, I agree that getting one from them would be helpful.

Yes get the pharm D rec! There is no problem with that imo as ID works pretty closely with pharmDs esp in abx stewardship.

Agreed. My interview season had several programs where I interviewed with pharmacists/Antibiotic Stewardship program peeps so I'm sure it'd lend a helpful hand.
