Center for Pain Management is excited to offer a multidisciplinary chronic pain fellowship opportunity in our multiple locations throughout Indiana. All our procedures including implants are performed in the center’s dedicated licensed ambulatory surgical centers. Our fellowship, although not currently ACGME accredited, will provide a diverse experience in the treatment of all spinal diagnoses, CRPS, fibromyalgia, cancer and non-cancer acute and chronic pain syndromes which will prepare the candidate for clinical practice and alternative board certification (other than the ABA exam). The program will focus on both the clinical evaluation and decision making necessary for appropriate patient selection and the procedural skills required to be technically proficient across all aspects of interventional pain procedures. The training will encompass diagnostic nerve blocks, CTL, ESI’s, MBB’s, RFA’s, SCS and DRG neuromodulation including full implantation of Leads and IPG’s, IT Pumps and the management of complex drug regiments and infusion therapy including sub anesthetic ketamine and lidocaine therapies to improve the quality of life for patients suffering from chronic pain.

The Center for Pain Management is a multidisciplinary and multi-location practice in Indiana. We were founded in 1992 by Dr. Edward Kowlowitz. Our physicians are both board certified in Anesthesiology and have added qualifications in pain management. In total, our physicians complete 50,000 outpatient visits, 4,000 new patient visits, 10,000 interventional procedures and placement of 400+ spinal cord stimulators annually.

Program Description:


Our philosophy maintains that responsible oral opioid therapy is an ethical responsibility of our discipline and that any physician identifying himself or herself as a pain management physician needs to provide, or oversee the provision of that service to his or her patients. All fellows will be trained in and will become proficient in evaluating and managing the medical and psychosocial aspects of pain, psychological aspects of chronic pain, and dual diagnosis of pain and addiction.

This fellowship will be for a period of 12 months. The fellow will be working under the supervision of each of our board-certified physicians on a rotational basis throughout the program to ensure maximum exposure to varied perspectives in the treatment of chronic pain conditions. Time will be divided approximately 50% completing interventional procedures and 50% in the clinical environment evaluating patients. They will work with our ancillary team including PhD psychologists, physical therapists, and Nurse Practioners and PA’s with extensive clinical experience, as well as complete a didactic program similar to the requirements of all ACGME programs.


This program is non-ACGME accredited but we shall be seeking accreditation in the future.

We expect successful fellows to have the ability to pass alternative pain board certifications other than the ABA, and would be strongly considered for employment at our existing facilities and/ or De novo practices commensurate with their abilities and expertise. The candidate is required to have a full unrestricted Indiana State Medical License and DEA number. Completion of residency in Anesthesiology or Physical Medicine and Rehabilitation is needed for the program. A salary will be provided for the course of the 12 month fellowship.
