Hi Everybody.

I am planning to take the NBDE II exam. Wanted to discuss some questions. Hopefully thru this we can help each other out.

Lets start:

Q1. Which antibiotic combination has the widest spectrum?
- Amoxicillin + clavulanic acid
- Pen V + tetracycline
- Amoxicillin + Metronidazole
- Erythromyicn + Tetracycline

i think its amoxicillin + clavulinic acid
 
even i think that its amoxicillin + clavulinic acid i.e AUGMENTIN ..it is a broad spectrum antibiotic which even reduces development of drug resistant becteria.

correct me if iam wrong

Ok..augmentin is a broad spectrum antib with activity against gram - and +....BUT not anaerobios. Metronidazol is for anaerobic bacterias, and spirokettas (sorry with my writting, english is not my tongue)


SO.....the correct answer is Amoxicilina+ Metronidazol, because is indicated against P. gingivalis and A. Actinomycetemcomit... in Adult and Refractar Periodontitis.

-Pen V+ Metronidazole. Pen V is a soft drug (oral), and Metronid has wide spectrum. And in this patient the origin of infection was probably periodontal (anaerobic bacteria)....This means that this combination for This kind of patient is the best.
-Clindamycin

Because the infection is severe we should prefer IV drugs. It can be Pen V oral(for Cocos gram+, gram-) and Metronidazol(IM) because has a wide spectr, for anaerobic bacillus gram-, and ,actinomicetemcomitans...Pen V +metronidazol is a better choice

using B speed film exposure to patient at 3 feet is 1 R. What would be the exposure be with D speed film at 6 feet?

ans is 1/8 R??
I started reviewing NBDE 2 stuff earlier today, so I will take a bite at this.

First of all, do the numbers for the B speed film using the Inverse Square Law.

Original Intensity/New Intensity = New Distance Squared / Original Distance Squared

So... 1 / x = 6 squared / 3 squared

find x (for new exposure or new intensity or R)

x = 1/4 (this is R assuming if the film speed was still B)

D speed films are faster and more sensitive than B by half, so the exposure conversion will be 1/2 x whatever B is. So 1/2 * 1/4 = 1/8.

I hope I am right~ :D

yes,i think that you can say both spenopalatine fossa or pterygopalatine fossa are correct. ..but more appropriate answer would be pterygo palatine fossa.

In mand nerve block ,the solution is deposited near the mand foramen... i dont think we deposit it into infratemporal fossa as it is a space behind the maxilla.
