Does anyone have a good narcotic conversion chart?

These charts vary considerably in dosage equivalency so should only be taken as a starting point. Titration to effect is the key. Also remember for the same drug being used in a population, there is a 4 fold variation in plasma levels for hydrocodone and oxycodone and an 8 fold variation in morphine levels.

i realize these charts are not 100% acurate - but as a good ballpark I would like to have a thourough equianalgesic chart/program to keep on my PDA. Seems the conversion chart that comes with the pdr program i use is limited -anyone have a good suggestions that they have used and like?

the one that is part of Epocrates is pretty good
 
duragesic.com has a good one that can be easily followed
