MD PhD school list help

I have already applied to 32 schools and am working on secondaries. Like any md-phd applicant, my list is top heavy and I am wondering if I should add in any more mid-tier schools.
522, 4.0 GPA, 2 years undergrad research, will do 2 years of postbac by the time I matriculate. Unfortunately covid and bad timing means I don't have any pubs yet, but my undergraduate research is the basis for a current post-docs project and I have plenty of conferences/oral presentations. My ECs include being an EMT and several leadership/volunteer activities. Im queer, so I have a good diversity essay but not technically URM I don't think.
