Two minor violations, both some time ago, shouldn't hurt that much. Your rejection rate will be 100% if you don't apply.
Technically, OP can't be rejcted if they don't apply in the first place. It's like dating. She can't say no if you never ask her out!

these are minor and I know people with actual criminal records who received multiple acceptances.... just be up front and honest... because if you don't and the school finds out you are toast.

idk, I wouldn't even report a freaking noise violation. That's not what med schools are looking out for.
