how can i differentiate between them since both have elevated pth and calcium
uworld say the difference is between urine calcium but since both have elevated calcium ?which appear in urine( i know hypocalciuric will not) but why? both have elevated ca

Primary PTH has high urine calcium + familial hypocalciuric hypercalcemia has lower urine calcium. It seems clear enough. Also as the name says, there is a familial history to the latter one. I think the way they would test this would be the mechanism of the latter one, which is calcium sensing receptor gene failure in parathyroid and kidney.
