How to list hours on AMCAS/secondaries

I have volunteered at a hospital for a few hours a couple times a month for a few years. I don't know how I should list this exactly. A lady at AMCAS said to just list how many hours i usually volunteer and in the explanatoin to say i do this a few times a month or semester and list the start and end dates. That seems reasonable. However, on secondaries do i list the same number of hours when they only ask for hours and dates? Will schools misinterpret this?
Also, we don't just take all the hours we did something and the start and end dates and avg. the hours right? I mean surely med schools realize you don't do something on campus during the summers but if you factored in all the summer months where you didn't participate in a student organization, it would really decrease the hours. Also, do we list future activities/hours? Let me know what you think.
