Many years ago, I attended a graduate school program that I thought would be a good fit but unfortunately was not. I tried to make it work in the program, but, at the end, I no longer wanted to be there and the faculty likely no longer wanted me there either.
There is a question on the APPIC application about if there has ever been any disciplinary action taken against me, now or in the past, in program. I do not recall if there was any specific action ever taking against me in my past program, but I also do not want to answer this question wrong or mislead anyone, because I don't want anyone to think I am trying to hide something or mislead anyone. How likely are sites to contact distant programs, supervisors, etc.?
Over 10 years? Probably unlikely that the old program will be contacted. If you are honest about it and convey your subsequent maturity, you will probably be fine.
