2 questions:

which is more common?
toxoplasmosis brain infections OR CNS lymphoma (AIDS pts)

Which is more common?
Septic joints OR Gout (old person ~55yo)

do you ever get septic great toe joints?

Septic joint = fever and high WBC count
Toxo = multiple lesions/ CNS lymphoma = single lesion

Doesn't matter which is more common, because they won't ask that.

And anything can become septic if you try hard enough.

1. If pt is taking bactrim prophy for PCP, CNS lymphoma would be more common. If not, toxo is the winner. Plus toxo presents with multiple lesions (as mentioned above) and CNS lymphoma will have positive EBV PCR on CSF analysis.

2. Gout by a long shot.

Primary B-cell CNS lymphoma in AIDS patients is usually multifocal and frequently indistinguishable from toxo on imaging.

Compared to Uworld, there were very few "which is most common" questions.

I just took it Thursday, and The majority of my questions were very basic, and rarely linked to any one identifying point. Very few fit any kind of tidy description of a disorder. It was bizarre how many were respiratory based, and how few were cardio based. Overally, ID was probably the most common denominator.
