Hospital narcotic discrepancies

Hello all,
Wanted to see what other hospital pharmacists do when narcotic discrepancies, caused by nurses themselves, are presented to you. At my hospital, the nurses want to pass the onus onto pharmacy, and my dop isn’t much help in giving direction. Thank you.

Who ever causes the descrepency needs to resolve it - often (at least in pyxis) the person who "discovers it" is not the person that made the mistake. Usually a charge nurse can do an investigation, and often the person who messed up has already finished their shift. But nursing is responsible for rectifying it (in pyxis and a variance report) - we have a rph who is assigned to follow up on all narcotic discrepencies to ensure they were resolved approrpaitely. Everything that fires in pyxis need to be resolved, and the resolution is audited appropriately.
