endocrine and international work

how possible would it be to specialize in endocrinology and then spend a considerable amount of time out of the country doing international public health and patient care? stuff like international relief work, refugee health issues, delivering care to very poor areas. it doesn't seem like one of those specialties where it'd be all that easy, like ID or ob/gyn, but i'm sure anything is possible. anyone have any thoughts on the matter..
