To comply with core measure VTE-5, we have created a warfarin tracker board, an electronic tool, to help the pharmacists identify all the patients in the hospital on warfarin, patients who need warfarin teaching prior to discharge, duplicate therapy and most recent INR. We are thinking of publishing an article to help other institutions implement something similar. Is this something you would be interested in learning about?
How are you currently identifying patients who need warfarin teach prior to discharge?

Our clinical pharmacists are pretty old school in how they monitor - all of the patient names are written down and followed, so I don't believe we need duplicate therapy or most recent INR tools. A coumadin education discharge que would be nice for after hours or on weekends.
