It is a case of 58 years old patient, that presented with a slow growing 5cm mass of her left posterior back, lying directly on posterior arcs of 10-11th rib, invading the skeletal muscle. It was asymptomatic.
She underwent a radical excicion.
Pathology report:
Desmoid fibromatosis, 6*5.5*1.5cm, invading the skeletal muscle, inked surgical margins are negative but less than 1mm posteriorly.

Do you offer adjuvant RT in this case?

I wouldn't. It's the first manifestation and a complete resection. Observe and reevaluate approach if/when recurrence occurs.

Agree- would treat only if it recurs.

Do you offer adjuvant RT in this case?

No. No role for adjuvant RT in desmoid R0 resections, and even R1 is debatable given that R0 and R1 have similar LR rates without adjuvant RT.

Also, surprised she went to surgery for an asymptomatic desmoid, especially without trying tamoxifen or NSAIDs first.
