Critical Care Practice Models

Hey folks

Just wondering about different CCM practice models. I am aware of Pulm/CC where the office/clinic work revolves around Pulm issues (i.e. Asthma, COPD etc...)

Are there any practice models that have private practice CCM? Do the CCM patients follow up with the CCM docs or do they follow up with their privates?

Are there models where folks do IM office based the time then CCM as well?
