Creatinine Clearance on Naplex?

At all the different institutions I have been at for rotations, each one calculates Clcr differently!!
For the naplex, should you use actual body weight or ideal body weight?

What if the patient is obese, do you calculate adjusted and use that?

The Commonly used formula for calculating Creatinine Clearance is Cockgroft-Gault equation which includes parameters such as age and weight of the patient, so even if the patient is obese, the weight of the patient parameter will take care of it.

Sometimes you use adjusted body weight or ideal body weight instead of actual or 1.0 instead of actual SCr, and I've had different experiences with these at different institutions. I haven't looked into what should be done for the NAPLEX yet though, so I can't help answer the question.
