The fact that you come from a T20 school alone puts you in a really good place.

I think as long as your letters are strong and you interview well, odds are probably pretty good.

T20 + decent Step 2 and you should be iight for what people consider 'prestigious' programs.

The Big 4 are typically extremely academic/research heavy so less likely, but research also doesnt seem to be your interest so that shouldn't matter as that programs probably aren't a great fit for you anyways.
