Hi,
Can anyone help me with any research/info out there on the accumulation of smegma in uncircumcised boys? While on peds, one of the patients had a collection of substance under his foreskin that caused him pain x1 week. Pt had hx of not retracting/cleaning under foreskin. We expressed alot of white, thick, liquid from around the foreskin, but could not retract foreskin secondary to pain. We had thought it might have been an abscess, but cultures came back negative, so we now assume it was smegma collection that liquified. Any budding urologists have insight or resources on subject?

Those were keratin pearls under his foreskin. Smegma forms in postpubertal boys/men.

It caused him pain likely because you were retracting his foreskin which had some adhesions to his glans.

These adhesions typically wear away as a boy begins to get erections.
