The difference in rank is not worth $250k to me. Both programs allow you to work with, or at least be close to, the place you want to be. As far as I know, while NYULI wants to train primary care doctors, that mission hasn't prevented past students who developed other interests from matching into whatever they want. As long as you think you will be able to explore enough and settle on a speciality within the first 2 years, NYULI all the way.
