What's the atmosphere in academic IM?

I would appreciate hearing from anyone about what it would be like to work in academic IM these days. Someone recently said to me they are becoming hospitalists with better schedules b/c of the teaching/bureaucracy-- he was jaded about being a faculty there after just a couple of years, and was going to go back for a fellowship, so I will take his opinion with a grain of salt. But what do you do if you don't want to go after a fellowship? How do you get involved in research if you want? How do you find a niche for yourself in a big department?
