In primary hyperparathyrodism there is increase in serum and urinary calcium. How FHH differs from it by having hypocalciuria? What is exact mechanism behind hypocalciuria?

CaSR, the calcium sensor in the kidneys registers low calcium, like it does in the parathyroid gland. The kidneys increase calcium reabsorption, leading to hypocalciuria.
