Take the guarantee and save yourself a bunch of headache getting in. From what I know the cutoffs were once 3.5 and 30 for the combined programs, and this is a far reach from the average statistics most people need to get into medical schools these days. The extra time you will have can be utilized to further your other interests, whether they be business, research, or whatever.

In addition, you're young, your interests might change, and 2 years down the road if you've done the pharma internship and not liked it you will kick yourself every day for not taking the guarantee while you had it.

Lastly, in my opinion, and this can be a controversial topic, but if the caliber of business school matters to you in your career path, that means you're gunning for IB, PE, or one of the other hyper-competitive business fields, and you shouldn't be going to medical school at all. You should be getting a degree in business and going to business school the traditional way. Your medical degree is going to consume you when you get there, whether you want it to or not the business degree will be complementary.
