UCLA maiden name & father's name

Anyone noticed that if you put name on one field (lets say father name, page 3), it will show up on another(maiden name, page 2)?

Or it is just me?

it did it for me too.. and i had to reprint it

I think it did it for everyone. That's how they made the .pdf. So I left it. I assume they either way it like that/or will notice everyone's is like that and ignore it.
