If life or personal desire take you away from academia, how realistic is it to have at least a side-gig doing something in the private world?

Depends on your interests & local market.

There's tox clinic, but typically needs to be linked to an OccMed setup as a lot of consulting comes from there.

There's industry - primarily pharmaceutical; and there's e medicolegal work. These can pay well, given the right circumstances.

And last but not least, always options with a poison center which, while not strictly private, is non-clinical for the most part.
 
Do people ever work as a consultant for the hospitals in their area or is tox consultation generally only an academic service?
 
Absolutely work as consultants, either for their hospital or system. The Kaiser toxic group has telepresence & virtual consults to cover the whole KP system, while most actually live/work in San Diego for example.

It need not be a purely academic pursuit; however, reimbursement is sometimes a sticking point with department heads, as an EP in the EDF can generate more revenue than a tox consult... so need to have a supportive boss. d=)
