I have a friend who may be interested. However, the form looks really shady. No space to put reference #3, and the whole form seems to be plagiarized from the APhA foundation? Or are you affiliated with the APhA foundation?

Hey, thanks for pointing out that oversight! The former executive director of APhA Foundation, Mindy Smith, is now with PrescribeWellness.

We've corrected the form. Also we just require 2 references now. Let me know if you have any questions.
