NYU IM Away Rotation

Hey ya'll!

I'm trying to do an away rotation, particularly in Electrocardiography, at NYU. I can't seem to find the address my form is supposed to be sent to, or who the student coordinator is, anywhere on their website. If anyone else has experienced this and has the solution, please let me know. I've been sitting on this letter for days and just want it out of here already.
