Does Step 3 score or In training exam matters when it comes to applying a fellowship (like geriatrics) after FM residency?

Do you need any LOR from medical school for your fellowship applications?

I am starting a FM residency.... Can anyone talk about how can I prepare myself during residency (score, research, rotation etc to be a better candidate for future fellowship? Thank you!

It's a factor, but I don't think it's too big of a factor. That being said some of the programs I applied to definitely asked for my Step 3 and ITE scores.
You do not need an LOR from med school.

My recommendation for fellowship
1. Being a good FM doctor in training. That speaks above all.
2. Do as much as you can in the field you are interested in. Go to conferences, research, posters, extra rotations in the specialty, etc.

FM fellowships are a waste of time, especially geriatrics. Why blow another year just for the privilege of earning LESS MONEY than everybody else?!?!?!? Wake up, dude.

Most importantly, most "fellowships" in family medicine are not like the typical subspecialists fellowships where you get boarded. There is use in the fellowships, such as sports, hospice, and OB as they usually widen your scope of practice, give you more experience with specific procedures, and make it easier to be hired in that specific field.

During residency if you plan on fellowships, I think it would be most important to do rotations in that field and get letters from MDs in that field.
 
I was never asked for ITE scores...in hindsight I am not sure if I even had to upload these. Having Step 3 done as soon as possible is a good idea, see if you can do it before residency starts so that you do not have to take down time or rotation study time away to prepare for it. As long as you continue to show improvement in your scores (shows you continue to study, learn and further your education) it will not hurt. Have an idea of what you want to do going in and plan to schedule appropriate electives (sports med vs OB vs inpatient vs geriatrics) late into 1st year and early into 2nd year. Try to get the appropriate LORs from physicians in that field by the mid to late 2nd year so that you have 3-4 (including your program director) ready for your fellowship application. Try to have everything for that application done and ready to submit on Day #1 of the application cycle.

You just need an LOR from your medical director in residency. Maybe your clinic director. Generally there are plenty of elective slots in residency, you may not feel the need for fellowship if you can get the rotations you want.
 
Should be ok for Geriatrics. Many spots go unmatched. As long as you passed everything, they won't care what your scores on USMLE was. Now, sleep and sports medicine are competitive FM specialities. So you have to score well and have good LORs to get in.
