Error types when reading and writing

Hi all, i am trying to understand some language problems in an elderly patient better.

Patient can spell words like cat and dog without difficulty but spells words like museum as 'munchum' and continuous as "continurus"

When reading, he read the word repugn as repung and the word schmear as "skuh-meer"

What types of errors is he making? is there a type of agraphia or alexia this would point more to?
I am not asking for a diagnosis, just for what best captures the nature of his errors.
