The EM forum would be more appropriate for this question. But to answer your question, anyone can do anything that they can convince a hospital credentialing committee to let them do - that doesn't mean it's a good idea. There are plenty of nephrologists, general surgeons, etc that moonlight in rural EDs due to the fact that EM has only had a formal residency training for a few decades and, historically, it has been specialists building a practice by referring to themselves while working in the ED. That being said, just because you can do something doesn't mean you should. Legally, you would be held to the same standard as a board certified emergency physician. Also, as EM programs are becoming more common, this is rapidly going the way of the buffalo.

What? Becoming a neurologist to work in the ED is a horrible idea. Why would anyone want to do this? If you want to work in the emergency department, please do a residency in emergency medicine.

There are some desperate rural EDs that will hire internists and family docs (definitely not neurologists) but as stated in the previous post, this is going away.

Can you do basic Gyn? PEDs? How are you at reducing fractures? These are all vital ER skills that are not stressed in IM or Neuro.

If I could get a job where all I did was manage the critically ill medical patients as they rolled in, I would take it in a heartbeat (being IM-CCM trained). But I doubt you'll find a job like that in 99.9% of EDs as you still need someone to care the traumas, peds, and vaginal bleeding, which is not in the internist wheelhouse.

I'll never figure out all these people wanting a back door into Emergency Medicine. If you truly want to be prepared to do both EM and critical care, the only way to do be properly trained for both is to do a critical care fellowship after EM residency.
