um... i applied to 50 programs for the categorical and I was under the impression that clicking the categorical box meant the first 2 years would be included.

are you saying that i have to click on the pediatrics specialty and look for the same school and apply to the pediatrics program again? thats idiotic. then why make a categorical a choice in the first place?

Most schools that have categorical positions dont require you to apply to peds...and if they do, it's the same application (charge ya extra). if it's advanced positions, then that is a different story. I only applied to categoricals. And I think 4 out of my 14 required an extra peds app.

i went to eras and chose just Pediatrics then the states where i just applied and under some of the programs it will say Child Neurology preliminary. I was going to go ahead and look to see if other programs I already applied to are like that.
 
I went ahead and emailed PCs of all the schools I applied to. .. especially the ones that didn't explain 1 vs 2 applications on respective websites. The program coordinators were all very nice and answered within an hour.

I went ahead and sent the same letters and personal statement. In a previous thread someone mentioned that they know were doing child neuro so it was best to keep things consistent. some of my letters were from general pediatricians anyway so i didnt think it was a big deal.

agree or disagree??

I think as long as your personal statement gives some insight regarding your desire to work with children, it makes sense to use the same for both applications.

I heard it was 15 or so. it seems most programs dont offer that many interview dates compared to other specialties. I dont know how its possible with the time constraint and fatigue to do more.

Thank you for the insight. I am trying to rearrange my schedule a bit to open up more time for interviews. 2 days of interviewing with a half day of travel will add up quick.

I'm already certain that I want to become a neurologist, but before this week I only seriously thought about adult neuro. Child neuro is a quite interesting new idea to me and I need to think about it through the next half year ...

My questions for now, if any of you could answer, are:

1, I'm limited in the geographic area for residency for personal reasons, and probably would only apply to about 5-8 schools, most of which in the top tier. I wonder if it's possible to apply to both adult neuro and pedi neuro for each of these schools? Would that be perceived as a lack of dedication and offend either side?

2, It seems like most folks chose pedi neuro starting from an interest in seeing children. My interest however mainly roots from neuro, and so far I can see myself doing either adult or pedi. I don't really find young children more attractive or annoying than the elderly. Do you think in terms of life-long career, this lack of specific interest in children would become a problem?

3, Is it going to be pretty much standard to do a 2-year fellowship, most likely in epilepsy, after the 5-year pedi neruo residency?

4, My home school does not have a pedi neuro elective. Is an away elective highly recommended or de factor mandatory?

1. Many of the pediatric neurologists I've spoken with were debating between child neuro and adult neuro in medical school. The most common deciding factors were physician/resident personality and tolerance for the bread and butter chief complaints (headache and abnormal movement/seizure in peds neuro vs stroke and peripheral neuropathy in adult neuro). Truthfully you will likely get a good deal of both in either field. You'll have difficult parents in peds neuro an difficult children and grandchildren in adult neurology!

Most of the large and "US news ranked" pedi-neuro programs are housed in free-standing hospitals with some affiliation to a neurology department in a university adult hospital. While there is a possibility you may come across the adult neurology program director on your child neurology interview day it is unlikely you will be interviewed by the director of the adult program on your pediatrics day nor is it likely the people making the decision to rank you are the same people. So far, I have only interviewed with pediatric neurologists.

2. The split is about 50-50. I liked neurology first. Keep this in mind when you ask folks about the required adult neurology experience during residency. I found that the residents who disliked the adult year were the ones who came to pedi neuro through finding pediatrics first. However this is just an observation. Much of pedi-neuro extends across the live course so if you want you can keep your patients well into their adulthood or for life. Get more data by working with infants/children/teens and elderly patients. I hope more exposure will help you make your decision.

3. This is changing. Many of the top programs have recognized that there is an increasing shortage of general pediatric neurologists. Programs are interested in folks who want to do general neuro and sub-specialty neurology. Fellowship length varies. Some are 1 year. Two year programs may include protected research time. From what I've seen in my cursory investigations, the length and content vary from institution to institution for example: some are fellowships are in epilepsy others include epilepsy within clinical neurophysiology.

4. I recommend doing an elective in pediatric neurology. I got interested in pedi neuro through an out-patient clinical experience. I fell in love with the field after an inpatient sub-internship. As with adult neurology, you will see the majority of your patients in the outpatient setting. In the inpatient setting you will see critically ill babies, kids and neurologically devastated children who may live for a long time but be totally dependent. It is tough to deal with seeing these things everyday for a month let alone 3 years. If your only exposure to neurology is in the inpatient setting, you may walk away with a skewed perspective of the field. As for doing away rotations, some programs like UCSF and Boston Children's say they strongly encourage an away elective. The field is not (yet) as competitive as derm or ortho so doing away rotations all over the country is not called for. If you have your heart set on a particular institution you can do your away elective/ sub-internship there and get a feel for the culture of the institution. The resident classes are so small that personality and fit really matter to the program directors and your future senior co-residents.

My question is how exactly does the fellowship work. I am an M3 who just started my pediatric clerkship. I have been wanting do do neurology, but I am interested in the pediatric part of the field. So, does everyone have to go through pediatrics? Is this a full 3 years of fellowship? I am guessing that many programs are different? Thanks for the help!

There are several different ways to go into Child Neurology. The most common for US grads is to do 2 years of general pediatrics, and then 3 years of child neurology; 5 total years. You can also do a full 3yr Pediatrics residency, and then apply into the Child Neurology programs. The Child Neurology training itself is standardized by the ACGME, and currently includes 1 full year of adult neurology, with the remainder being 2 years of child neurology. There are certain rotations that are required, but again, these are all standardized between programs. There is, however, a lot of discussion within the child neurology community regarding the amount of adult neurology that should be required, and so by the time you apply to child neurology, there may be less adult neurology time required. However, at this point in time, the pathway is as I described above: 2 years of general pediatrics at minimum; 3 years of child neurology, which encompasses 1 full year of adult neurology.

So this thread was really created to help current medical students (and residents) with the child neuro residency application process, but I'll respond to your question. Short answer, yes, just email her, the worst case scenario is that she doesn't write back.

Longer answer- getting a job working with special needs children may actually give you more bang for your buck at this point. Or a job at the children's hospital- I think that being a scribe could be a neat way to put yourself in the midst of patient activities while also having an active role. Maybe get involved in some clinically-oriented research, where you could gain some knowledge and skills. Shadowing is a way of getting patient exposure, but it's so passive and depends on how much time/attention span the preceptor has to actually explain the thought processes behind her decisions. This can be tough to balance in a busy clinic.

I am a third year medical student. I want to go into child neurology but I am not really sure of the process. Do I need to rotate at sites I plan on applying to residency at? Is it competitive? I'll bite- you don't necessarily need to do away rotations, but if you have a dream program in a specific location, then it'd be helpful. It is not competitive like derm or ortho or anything like that, but getting categorical spots in some of the top programs can be competitive. I've certainly met DOs in child neurology residency.

How many to apply to kind of depends on your goals- are you determined to match in a super desirable location, or something like that? Generally, I'd say apply to around 10 (I think that's how many you can apply to for a fixed cost, then more $$ after that), and include programs with a range of competitiveness. If you're nervous, apply to more, and you can always turn down interviews later.

What can you do now? Do your best this year, especially in your peds and neurology rotations. Everything else is advice for any specialty- do well in your 4th year rotation as well, get step 2 out of the way, then interview and see what happens!

From this limited information, I can't imagine any specialty that you wouldn't be competitive for! See my post above.

Sub-I timing- generally you want these done early enough to have a faculty letter ready by interview time. So, October at the latest. I did my sub-I in August and away rotation in October, to give you an idea.

Also- no research is ok. Just show what you're passionate about in your essays.
 
So how competitive are the reserved positions? I'm a pedi resident and I came to child neurology late :(. I'm seriously considering it but there are so few reserved positions, that I'm worried about finding a spot at a good program.
 
You should go for it! The number of reserved spots really varies from year to year. Right now, 3 of our 5 junior residents got reserved spots after 3 years of pediatrics. It's kind of a crap shoot, depending on how many people drop out of their advanced spots (like if they just finish pediatrics instead).

For the sake of collective knowledge, the CNS posts all open spots on a yearly basis for CN and NDD. look at what's available and touch base w/the programs. There's dozens of spots but the availability may vary as mentioned below (others straight up have reserved spots always available). Last year was a great year in terms of options out there. This year may be different or the same. I don't know.and fwiw i only knew of 4 of us applying after a peds pgy3 last year (for a dozen or so spots I was aware of).

I was wondering about compensation for Peds Neuro fellows? Is there a big difference in salary between Electrophysiology/Sleep/Neuromuscular/Epilepsy/etc?

Is it even worth doing a fellowship? I was told that if you get enough experience during residency that can be enough.
 
Hi all! I'm a MS3, strongly considering Peds Neuro!! I'm at a school without a peds neuro residency program, so would really appreciate more info from you all re competitiveness, applying, aways etc.

My scores/stats:
- Step 1 = 221
- Honors in Peds clerkship and neuro elective
- 2 publications; neuro related research with prominent mentor; several poster and orals presentations; conference poster and participation
- Clerkship evals all strong and positive (usually shelf is my limiting factor for honors...)
- Other = distinction in 3 preclerkship classes, served as med school tutor, GHHS, CA med school with top 50 NIH funding

1. What is my competitiveness? Would love to match at UC school.
2. How do I become a stronger applicant? Do you recommend taking a research year? What can I do to show I'm capable, given my low-ish step score?
3. I'm thinking of doing 2 aways during 4th year to solidify my decision/meet faculty and residents in the field etc. What do you know about the culture of the different California Peds Neuro departments?

It sounds like you should have a pretty good shot. I don’t think it’s worth taking a year off- I think that doing a good job on your away rotations will be the biggest thing you can do to help land a spot in Cali. Definitely do the aways after your peds sub-I, and certainly after your MS3/4 neuro rotation.

I had done an away at Boston Children’s and I think that definitely helped me meet everyone and ultimately match there. Be sure that you find an opportunity to present a case/topic to the group- I presented a moya moya case that was on service while I was there (along with new research findings on moya moya, etc).

I think it’s the same as with any other specialty- take it early (I.e. score available by Sept 15) if you did lower than you wanted on Step 1, to show your improvement. Otherwise, just get it done so it shows up before rank lists are made.

Any updates on salary and Match competitiveness?

I’m now in the job search and can comment on salaries- in academics, if you subspecialize in epilepsy, you can start at $200K+ (depends on the part of the country, maybe +/- $50K). Without epilepsy fellowship (so gen peds neuro), starting salaries are maybe 40-50K lower.

Private practice, especially in a smaller town, can be $100K more than the numbers above. (Based on the recruiter emails that I get)

Any updates on salary and Match competitiveness?

Re: match competitiveness: as the above shows, it's noted as the specialty with the highest match rate (99%, with only 1 US senior not matching). PDs I've spoken to (both at my home institution and at interviews) still emphasize that there are more spots in child neuro than there are qualified applicants. So my perception is that it's still not a very competitive field to match into.
 
Curious- do pediatric neurologist see adult patients at all? Are they board certified in adult neurology?
