My ICU fellowship at University of Michigan was the single best year of my medical education. If you'd consider Midwest programs, I'm biased, but I think UMich is worth a look.

Columbia best in nyc imo, mgh/BID/Brigham all solid. Varying opinions on which of those boston programs is best

I dunno if you consider UPMC Northeast but it's a pretty good program. PM me for details.

Consider Emory. The CCM fellowship here is well-rounded with exposure to medical patients all around. Plus we manage severe ARDS and ECMO in our units.
